I am writing this letter to humbly request your permission for a one-day leave from school to visit an optometrist for an eye check-up. I have been facing some health issues related to my vision, which have been causing difficulties in my studies.

For the past month, I have been experiencing trouble seeing what’s written on the board clearly, even from the front row. As a result, I have been straining my eyes, and it has been challenging to keep up with the lessons. Moreover, I have been suffering from minor headaches, which I believe are connected to my vision problems. I have been experiencing nausea, especially during the school bus journey. The constant motion combined with my blurry vision has made it quite uncomfortable for me to travel to and from school.

I sincerely request your understanding and permission to leave school on Monday, July 24 to visit an optometrist for a thorough examination. I assure you that I will make up for any missed classwork and homework during my absence. I will also provide you with a copy of the doctor’s note.
