7. Compose a dialog between a clerk in a hospital and a patient about taking an appointment with a doctor. Write at least six exchanges. [6]
(A patient goes to the hospital.)
Clerk: Good morning! How can I help you?
Patient: Hi, I need to see a doctor. Can I get an appointment?
Clerk: Of course! May I have your name and date of birth, please?
Patient: Sure. It’s Nisha Tuladhar, born on June 10, 2002.
(The clerk types the information into the computer.)
Clerk: Thank you, Ms. Tuladhar. What’s the reason for your visit?
Patient: I’ve been having headaches lately, and I want to get it checked.
Clerk: I’m sorry to hear that. Let me check the schedule. How about tomorrow at 2 pm?
Patient: Oh, is there an opening today? I’d like to get is checked today if possible.
Clerk: Just a minute. Let me check. We can do at 5 pm today.
Patient: Sounds good. Please sign me up for 5 pm. I will come back later.
Clerk: Here’s your appointment card. Please bring this with you later.
(The clerk hands over the appointment card.)
Patient: Thank you!
Clerk: Is there anything else?
Patient: Nothing. That is all.
Clerk: Take care and see you later.
